Your first visit matters most. We, at the Metropolitan Foot & Ankle Group, ensure to give you only the best service that will mark a good impression that will last along with the help of our friendly staffs. Our mission is to provide comprehensive, evidence-based medical and surgical treatment of the foot and ankle for both adults and children.
Prior to your visit, we encourage you to call us to schedule an appointment or use our Appointment Request Form.
What to Expect:
During your first visit, we will obtain important background information such as your personal information, medical history, and a time to get to know more about your doctor.
Here are some tips for a successful consultation that will also save you time:
- Show up at least 30 minutes early.
- Fill out our New Patient Registration Form (you can click the link and it will redirect you to the form).
- You may fax the NPRF to our clinic or bring them with you on your visit.
- Bring your insurance card as we need a copy of it for our records. If your insurance plan requires that you have a referral form from your primary care doctor, make sure to obtain one before your appointment date.
- Bring at least 1 valid ID for verification purposes.
